You will not be permitted to race without a properly completed waiver form. Print it, fill in the blanks and sign it. Please note that registration is not complete until payment of the registration fees and we receive the waiver. Parental Authorization & Waiver of Liability
By signing this form, I (parent/guardian of the above named), affirm that I am the parent or legal guardian of (swimmer's name), and I agree, warrant and covenant as follows:
1. I give permission for my child to participate in the Beach-to-Beach Power Swim.
2. I understand that participating in this event is potentially hazardous, and that my child is medically able and properly trained for the event that my child has entered.
3. In consideration of the acceptance of this entry, I assume full and complete responsibility for any injury or accident, which may occur while my child is traveling to, from or between event venues, during the event, or while my child is on the premises of the event.
4. I also am aware of and assume all risks associated with my child’s participating in this event, including but not limited to falls, contact with other participants, effect of weather, boat traffic, and sea conditions.
5. I, for my child, myself and my heirs and executors, hereby waive, release and forever discharge the Friends of Virgin Islands National Park, sponsors, promoters and each of their agents, representatives, successors and assigns, and all other persons associated with the event, for my all liabilities, claims, actions, or damages that I may have against them arising out of or in any way connected with my participation in this event.
6. I understand that this waiver includes any claims, whether caused by negligence, the action or inaction of any of the above parties, or otherwise.
7. I understand that the entry fee is non-refundable and non-transferable. I hereby grant full permission to any and all of the above parties to use my child’s image in photographs, videotapes, motion pictures, website images, recordings or any other record of this event.
